Responsibilities

Registered Nurse Case Manager works in collaboration with the Interdisciplinary Discipline Team (IDT) to promote a quality experience while following the patient and family's care plan and wishes. The RN Case Manager manages symptoms of the patient's disease and re-evaluates effectiveness of interventions and adjusts the care plan as needed.

 

Admissions Nurse in a home setting, provides and arranges for hospice care for patients and their families in collaboration with members of the Interdisciplinary team.  The Admissions Nurse makes initial nursing assessments and treatment decisions, using their individual skills, based on patient and family needs and wishes. 

 

Registered Nurse (RN) Nights & Weekends Home Care must be prepared to handle all on-going needs, in addition to urgent needs of patients and their families after hours whether in their homes or in an inpatient facility.  The established care plan of the patient will be followed while insuring comfort care to the patient and support to family members.

 

Clinical Liaison serves as the first introduction to hospice and palliative care for a hospitalized patient and their family.  The Clinical Liaison educates the patient and their families on Hospice care and provides ongoing support. Together with the patient, family and physician, the Clinical Liaison aids in developing a care plan according to their wishes. The Clinical Liaison initiates the assessment to ensure that the care plan is appropriate and is followed.

 

Registered Nurse PRN - Hospice Inpatient Unit initiates and maintains the care plan for patients in the acute care setting based on decisions and wishes that have been established by the patient and their families. The RN-IPU focuses upon symptom management while addressing all other health care needs.  The RN- Hospice IPU works closely with the inpatient team to provide support to patients and their families.

 

Registered Nurse-PRN works as required based upon the scheduling needs of the team to provide care and support to patients and families. The RN-PRN insures that the patients care plan and wishes are being followed.  This position works in partnership with the IDT team and treats all healthcare needs of the patient including managing symptoms and providing treatment.

 

Qualifications

Experience Requirements
Must have one year of staff nurse experience in a comparable setting/unit. Hospice and/or palliative care experience is strongly preferred.

 

Education Requirements
Must have a degree in Nursing from an accredited nursing program. A BSN degree is preferred.

 

Required Certificates and/or Licenses
Must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice.  Licensure from compact state is acceptable. Healthcare Provider BLS (CPR) is required to practice in the State of Maryland and the District of Columbia. A CHPN (Certified Hospice and Palliative Nursing) certification is a plus.

 

Driver's License

Must have a driver's license in good standing and access to a vehicle during working hours.
