The Hospice Social Worker home care team provides a wide variety of direct and indirect social work services to patients/caregivers/families in accordance with the individualized plan of care, as prescribed by the physician. The Social Worker participates in ongoing interdisciplinary comprehensive assessments, develops and evaluates the Plan of Care, contributes to patient and family counseling and education and participates in the QUAPI program and hospice sponsored in-service training.
Schedule: Full time - days Monday-Friday
Location: Washington, D.C.
COVID-19 vaccine required to be completed upon start.
The Social Worker actively participates in coordination of all aspects of the patient’s care, in accordance with current professional standards and practice.
Experience Requirements
Will accept new MSW graduates as well as seasoned Social Workers with two years of master’s level experience in an interdisciplinary health care setting.

Education Requirements
Must have a Master's degree in Social Work from a school accredited by the Council on Social Work Education.
Licensing Requirements
License or license eligible is preferred but will accept new graduates with MSW.
Driver's License
Must have a driver's license in good standing and access to a vehicle during working hours.
