The Hospice Clinical Liaison (RN) serves as the first introduction to hospice and palliative care for a hospitalized patient and their family. The Hospice Clinical Liaison educates the patient and their families on Hospice care and provides ongoing support. This person triages all incoming referrals and coordinates interdisciplinary team members required to best meet the immediate needs of the patient. Together with the patient, family and physician, the Clinical Liaison aids in developing a care plan according to their wishes. In addition, he/she will initiate the assessment to ensure that the care plan is appropriate and is followed.

The Hospice Clinical Liaison initiates the plan of care, discharge plan and communicates with the interdisciplinary team regarding patient goals and treatment plans. The Clinical Liaison makes nursing assessments, judgments and treatment decisions based on patient needs and wishes. He/she promotes Capital Caring and Capital Palliative Care Consultants programs to acute care facilities through collaborative relationships with the facility by providing opportunities for professional development and community education. May be asked to rotate to other regions/departments
Experience Requirements
Must have one year of experience in a comparable setting/unit. Hospice and/or palliative care experience is strongly desired
Education Requirements
Must have a degree in Nursing from an accredited nursing program. A BSN degree is desired.
Required Certificates and/or Licenses
Must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice. Licensure from compact state is acceptable.
Driver's License
Must have a driver's license in good standing and access to a vehicle during working hours.
