The Hospice Social Worker provides a wide variety of direct and indirect social work services to patients/caregivers/families according to their individualized plan of care, as prescribed by their physician. The Hospice Social Worker actively participates in coordination of all aspects of the patient’s care and participates in ongoing interdisciplinary comprehensive assessment, develops and evaluates the Plan of Care, contributes to patient and family counseling and education and participates in The QAPI program and hospice sponsored in-service training.

Experience Requirement
Will accept new MSW graduates as well as seasoned Social Workers with two years of master’s level experience in an interdisciplinary health care setting.
Education Requirements
Must have a Master's degree in Social Work from a school accredited by the Council on Social Work Education.
Licensing Requirements
Licensed or license eligible in compliance with jurisdiction of practice (LCSW, LICSW) preferred, but will accept new graduates with MSW.
Driver's License
Must have a driver's license in good standing and access to a vehicle during working hours.
