Overview

The Resource Nurse is responsible for assisting the Clinical Manager in the daily triage of the Team activities of accepting referrals, assuring timely admissions, assigning patients to the appropriate team members and monitoring patient care as ordered on the interdisciplinary plan of care. 

 

Location:  Prince William County, VA

Responsibilities

The Resource Nurse is responsible for assisting the Clinical Manager in the day-to-day triage of the Team activities of accepting referrals, assuring timely admissions, assigning patients to the appropriate team members and monitoring patient care as ordered on the interdisciplinary plan of care. The Resource Nurse participates in IDT meetings and assists in reviewing and completing the IDT meeting notes. The Resource Nurse assists with medical record audits to assure quality care and compliance with state and federal regulations, CHAP accreditation standards, and the organization’s policies and procedures.

Quality Provision of Services:   

  1. Receives/triages inquiries from the Referral Center, staff, patients, caregivers, referral sources and the community. Performs telephone assessment to determine needs of the caller. Develops and implements a plan to address the needs by (but not limited to): instruction, dispatching appropriate staff to make a patient visit, or contacting a physician.
  2. Assists the Clinical Manager and Regional Executive Director to act as a liaison between Hospice and other professionals, including the AMD, community physicians, hospitals, nursing homes, and agencies.
  3. Enhances the integrity and clinical practice of the team by providing them with a consistent role model and resource person.
  4. Assumes responsibility for own professional growth and development in order to maintain and improve competence.
  5. Evaluates own needs for support and uses identified system(s) to meet the need.
  6. Develops new skills by participating in ongoing education and maintaining knowledge of current nursing practice through journals, literature review, etc.
  7. Participates in quality improvement activities, QUAPI program and hospice sponsored in-service training. Serves on committees or teams as approved by the Clinical Manager/Regional Executive Director.

Qualifications

Experience Requirements

Must have three years of Hospice/Home Health experience. A minimum of two years of related management experience and at least four years of medical surgical and/or oncology is preferred.

 

Education Requirements

Must have an LPN education from an accredited nursing program. A BSN RN is preferred.

 

Required Certificates and/or Licenses

Must be licensed as a nurse with licensure from the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia. 

 

Driver's License

Must have a driver's license in good standing and access to a vehicle during working hours.
