Hospice Social Worker - The Social Worker provides a wide variety of direct and indirect social work services to patients/caregivers/families in accordance with the individualized plan of care, as prescribed by the physician.  The Hospice Social Worker actively participates in coordination of all aspects of the patient’s care, in accordance with current professional standards and practice.  The Social Worker participates in ongoing interdisciplinary comprehensive assessment; develops and evaluates the Plan of Care; contributes to patient and family counseling and education and participates in the QUAPI program and hospice sponsored in-service training.    Schedule: full time, Monday-Friday   Location: Prince William and Fauquier Counties, VA   COVID-19 vaccine required to be completed upon start.

Under the supervision of a Registered Nurse, an Inpatient Unit (IPU) Licensed Practical Nurse (LPN) provides the necessary treatments, education, and documentation consistent with the inpatient unit environment care. Certified Nursing Assistant (CNA-PRN) - Hospice IPU provides a central role in the care of the patient and works to enhance quality of life, in addition to assisting with all activities of daily living. Registered Nurse (RN) Hospice Case Manager works in collaboration with the Interdisciplinary Discipline Team (IDT) to foster a quality experience in accordance with the patient and family’s care plan and wishes. The Senior Accountant gathers and analyzes financial information and ensures accurate maintenance of the general ledgers of the parent company.
