Hospice Clinical Liaison serves as the first introduction to hospice and palliative care for a hospitalized patient and their family.  The Hospice Clinical Liaison educates the patient and their families on Hospice care and provides ongoing support. Together with the patient, family and physician, the Clinical Liaison aids in developing a care plan according to their wishes. He/she initiates the assessment to ensure that the care plan is appropriate and is followed.  In addition, the liaison triages all incoming referrals and coordinates interdisciplinary team members required to best meet the immediate needs of the patient.    Schedule: Monday - Friday 8:30am -5:00pm   Location: Montgomery County, MD   COVID-19 vaccine required to be completed upon start  

Quality Assurance Nurse is a Registered Nurse who is responsiable for ADR submissions, QA review activities, and regulatory compliance for all levels of Hospice care in conjunction with the QA Director.  The Quality Assurance Nurse (QARN) is responsible for assisting in the development, implementation and evaluation of quality assurance, performance improvement, and utilization review activities as requested by the VP of Organizational Performance/Chief Compliance Officer and the Director of Clinical Performance.   Location: Falls Church, VA   Hours:  Monday - Friday 8:30am - 5:00pm   COVID-19 vaccine required to be completed upon start

Hospice Registered Nurse-PRN works as required based upon the scheduling needs of the team to provide care and support to patients and families. Registered Nurse (RN) Hospice Inpatient Unit initiates and maintains the care plan for patients in the acute care setting based on decisions and wishes that have been established by the patient and their families. The Volunteer Services Administrative Assistant works in collaboration with the Manager of Community & Volunteer Engagement to help manage the daily activities as well as ongoing projects and goals of the volunteer services team. The Registered Nurse (RN)-Nights & Weekends is prepared to handle all on-going needs, in addition to urgent needs of patients and their families after hours in their homes or facility.  The RN Nights & Weekends follows the established care plan of the patient while ensuring comfort care to the patient and support to family members.   Location: DC, Largo MD   Schedule: Full-time:  7 shifts per 2 weeks. The Health Information Management (HIM) Coder is responsible for assigning procedures and diagnostic codes to patient records, including all primary and secondary diagnosis(s) for all patients admitted to Capital Caring Health.  Using different forms of coding libraries, the HIM Coder will also routinely monitor all active records and update them in accordance with state and federal guidelines, accreditation standards, as well as Capital Caring's Policies and Procedures.   Location: Falls Church, VA   Hours: Monday-Friday:  8:00am-5:00pm   COVID-19 vaccine required to be completed upon start.

The Housekeeper supports a clean and comforting atmosphere in administrative and inpatient buildings, which includes cleaning a patient's rooms, nursing stations, family and public areas, bathrooms, and kitchens.   Schedule: Monday - Friday, 8:30am-5:00pm   Location: Adler Center - Aldie, VA   COVID-19 vaccine required to be completed upon start.

The Maintenance Assistant will assist the facility manager in the Building and Grounds Department for The Adler Center along with their grounds.  This includes helping to make sure all systems and equipment are fully functioning and that these properties are well maintained.  Assistance with issues at the Merrifield Office as well as the DC and MD inpatient units is sometimes needed as well.   Location: Adler Center - Aldie, VA   Hours: Monday - Friday 8:30am - 5:00pm   COVID-19 vaccine required to be completed upon start.

Supplemental Staff - Registered Nurse works as required based upon the scheduling needs of the team to provide care and support to patients and families. The Hospice RN ensures that the patients care plan and wishes are being followed.  The RN works in partnership with the IDT team and treats all healthcare needs of the patient including managing symptoms and providing treatment.  This role is a temporary assignment, up to 12 weeks in length. Pediatric Registered Nurse Case Manager works in collaboration with the Children's Program IDT team to foster a quality experience while following the patient and family's wishes and plan of care.
