The Care Navigation Nurse Practitioner is an active member of Capital Caring’s Medical Staff and provides phone-based care management to our patients and families throughout our continuum of care. This includes, in part, home and facility-based hospice, home and facility-based palliative care, inpatient hospice, hospital-based palliative care consultation and outpatient palliative care clinics and Primary Care at Home. He/she provides clinical support to the Care Navigation (CN), to Triage Staff and Interdisciplinary Teams (IDTs) during evening and weekend hours to provide support to patients and families discharged from the hospital. The CN Nurse Practitioner will develop methods to improve clinical outcomes which ensure the continued quality of care, as well as guarantee timely communication of evening and weekend phone-based care management activity.

The Unit Secretary is responsible for providing administrative and health information management support to the Inpatient Center (IPC). The secretary may assist with insurance documents, personal information, patient rights and privacy information, as well as discharge and transfer paperwork. This person will be responsible for daily reporting of all patient status changes, database/spreadsheet updates, and assembly and analysis of all active and closed medical records.

The Hospice and Palliative Medicine Physician is a member of the active medical staff of Capital Caring Health and provides services to our patients throughout the course of their care. This person has advanced knowledge and skills to help prevent and relieve suffering some patients experience with life-limiting, life-threatening and terminal illnesses. The care provided includes, but is not limited to, home and facility-based hospice, home and facility-based palliative care, inpatient hospice, hospital-based palliative care consultation, and outpatient palliative care clinics.

Attorney - Healthcare Associate works under the direction of The Chief of Strategy, Policy & Legal Affairs, the Health Care Associate Attorney will assist with a wide variety of the legal matters for Capital Caring Health. This will include bringing their legal background to support the organization’s development and participation in value based health care arrangements. Activities include, but are not limited to, contract negotiation and management, preparation of innovation health care agreements, compliance, review of healthcare policy and regulations, real estate, and other matters pertaining to the daily operations of Capital Caring Health

The Admissions Nurse in a home setting, provides and arranges for hospice care for patients in a collaborative relationship with members of the Interdisciplinary team. The Admissions Nurse makes initial nursing assessments and treatment decisions based on the needs and wishes of the patient and their family. In addition, The Admissions Nurse gathers important contact information, reviews and documents the patient’s medication list and takes their full medical history.

Hospice Licensed Practical Nurse(LPN) supplements the nursing care of the patient as provided by the Registered Nurse (RN) and as specified in the patient's plan of care. The Hospice LPN provides nursing care as well as patient/family education and documentation under the supervision of a Registered Nurse. The LPN assists the physician or RN in performing specialized duties or procedures in relation to end of life care.

The RN Care Manager coordinates care for a caseload of primary care patients using phone or telehealth platforms. Working in collaboration with other members of the care team, The RN Care Manager assesses the patient's needs and develops individualized care plans in alignment with patient goals for their care. The RN Care Manager provides education to patients and their families/caregivers on how to manage chronic conditions, medication usage, and reporting symptoms needing early intervention.

The Nurse Practitioner (Primary Care) provides quality patient care in a variety of settings in collaboration with and under the direction of the Attending Physician. The Nurse Practitioner follows established nursing and organizational standards, procedures, and practice.
