Hospice Clinical Liaison serves as the first introduction to hospice and palliative care for a hospitalized patient and their family.  The Hospice Clinical Liaison educates the patient and their families on Hospice care and provides ongoing support. Together with the patient, family and physician, the Clinical Liaison aids in developing a care plan according to their wishes. The Hospice Social Worker provides a wide variety of direct and indirect social work services to patients/caregivers/families in accordance with the individualized plan of care, as prescribed by the physician.  The Hospice Social Worker actively participates in coordination of all aspects of the patient’s care, in accordance with current professional standards and practice.  The Social Worker participates in ongoing interdisciplinary comprehensive assessment, develops and evaluates the Plan of Care, contributes to patient and family counseling, education and participates in the QUAPI program and hospice sponsored in-service training.    Schedule: Monday - Friday 8:00am - 5:00pm  On-call weekend/evening TBD   Location: Largo, MD   COVID-19 vaccine required to be completed upon start

The Admissions Nurse in a home setting, provides and arranges for hospice care for patients in a collaborative relationship with members of the Interdisciplinary team.  The Admissions Nurse makes initial nursing assessments and treatment decisions based on the needs and wishes of the patient and their family.  In addition, The Admissions Nurse gathers important contact information, reviews and documents the patient’s medication list and takes their full medical history.   Location: Washington, DC   Hours:  5 8 hour shifts, set or varied schedules available. Nurse Practitioner –is a member of the active medical staff of Capital Caring Health and provides services to our patients throughout our continuum of care.  This person has advanced knowledge and skills to help prevent and relieve suffering some patients experience with life-limiting, life-threatening and terminal illnesses. Physician - Home Based Primary Care will join an established house calls practice providing home based primary care to patients who are home limited.  Care will be provided in independent homes and senior communities (including both assisted living and independent living.)  The applicant will join an established team consisting currently of 2 physicians, 2 nurse practitioners, social worker, triage RN, 2 office managers.  Clinical care will be a combination of home visits, telemedicine, and coordination of care across care settings.  Ideal candidate will have interest in care for frail elders with advanced illness.  Patient population is centered in Northern Virginia.     Location:  Headquarters - Falls Church, VA   Schedule:  Monday-Friday - 8:30 AM to 5:00 PM   COVID-19 vaccine required to be completed upon start.

The Registered Nurse (RN) - Nights & Weekends is prepared to handle all on-going needs, in addition to urgent needs of patients and their families after hours in their homes or facility.  The RN or LPN Nights & Weekends follows the established care plan of the patient while ensuring comfort care to the patient and support to family members.  The RN Nights & Weekends arranges for, teaches, and supervises nursing care of patients in home care and facility settings in collaboration with other members of the interdisciplinary team.   Location: Montgomery County, some PG county and DC coverage   Schedule: 7 shifts per 2 weeks, rotates nights and days.
