The Registered Nurse (RN) Hospice Case Manager works in collaboration with the Interdisciplinary Discipline Team (IDT) to foster a quality experience according to the patient and family’s care plan. The RN Case Manager plans and manages the nursing component of the hospice caseload. The RN Case Manager makes initial and ongoing nursing assessments, judgments and treatment decisions based on patient and family/caregiver needs. He/she will manage symptoms of the patient's disease and re-evaluate effectiveness of interventions and adjust the care plan as needed.
Location: Washington, D.C. - Home Care
Hours: Full-time: Monday - Friday 8:30am-5:00pm

The Registered Nurse (RN) initiates and maintains the care plan for patients in the acute care setting based on decisions and wishes that have been established by the patient and their families. The RN focuses upon symptom management while addressing all other healthcare needs. The RN works closely with the inpatient team to provide support to patients and their families.

Hours: Monday-Friday 8:30AM-5:00PM, 1 minor, 1 major holiday and on call for emergency coverage

The Hospice Licensed Practical Nurse (LPN) supplements the nursing care needs of the patient provided by the Registered Nurse (RN) and as specified in the plan of care. In addition the Hospice LPN assists with patient and family education and documentation under the supervision of a Registered Nurse. The LPN assists the physician or RN in performing specialized duties or procedures in relation to end of life care.

The Hospice Social Worker home care team provides a wide variety of direct and indirect social work services to patients/caregivers/families in accordance with the individualized plan of care, as prescribed by the physician. The Social Worker participates in ongoing interdisciplinary comprehensive assessments, develops and evaluates the Plan of Care, contributes to patient and family counseling and education and participates in the QUAPI program and hospice sponsored in-service training.
Schedule: Full time - days Monday-Friday
