The Registered Nurse (RN) Hospice Case Manager works in collaboration with the Interdisciplinary Discipline Team (IDT) to foster a quality experience according to the patient and family’s care plan. The RN Case Manager plans and manages the nursing component of the hospice caseload. The RN Case Manager makes initial and ongoing nursing assessments, judgments and treatment decisions based on patient and family/caregiver needs. He/she will manage symptoms of the patient's disease and re-evaluate effectiveness of interventions and adjust the care plan as needed.

The Hospice Social Worker home care team provides a wide variety of direct and indirect social work services to patients/caregivers/families in accordance with the individualized plan of care, as prescribed by the physician.  The Social Worker participates in ongoing interdisciplinary comprehensive assessments, develops and evaluates the Plan of Care, contributes to patient and family counseling and education and participates in the QUAPI program and hospice sponsored in-service training.    Schedule:  Full time - days  Monday-Friday
